Job Description

RN needed to work in outpatient practice/ STI clinic in Reading, PA. This position will include screening for and treatment of STIs, HCV, HIV.

Greet and acknowledge all patients according to agency standards

Receive, record and relay messages accurately, completely and legibly

Speak with others using clear, professional and polite language

Develop and maintain positive working relationships with others, support the ACG team to reach common goal

Work with the health care team to test, assess and treat patients for STIs, HCV, PrEP in collaboration with a supervising physician

Provide patient education one-on-one or with family members / support persons based on need

Document appropriately and timely in the medical record

Maintain current knowledge of STI treatment and cure

Confer with health care team regarding adherence issues, insurance concerns, side effects of medications as well as the client’s responsiveness to treatment

Provide consultation to other staff regarding areas of expertise as required, including nursing evaluations

Refill medications as needed

Immunize patients as needed and appropriate

Provide training to staff within organization as required

Participate in case conferences and staff meetings

Other duties as assigned

Requirements/ Qualifications:

RN or greater clinical license with 3-5 years’ experience
Experience in assessments, care coordination /case management, disease management, mental health and substance abuse and patient education
Experience or familiarity with assessment and treatment of STIs
Ability to perform comprehensive nursing assessment, problem identification and care plan development, including screening for health issues that may require specialty referrals
Experience with documentation in electronic health records
Experience with an urban health care center environment
Organization skills and ability to be resourceful self-starter; strong ability to work as a member of the health care team
Excellent written, oral and interpersonal communication skills
Spanish language skills strongly preferred
