Position Title: Residential Case Manager

Reports to:  Assistant Director and Division Director – Welcome Center Services Division

Category: Exempt

Location: Friendship Place Welcome Center (4713 Wisconsin Ave NW, WDC 20016)

Hours: Monday – Friday (8:30 a.m. – 4:30 p.m); Some evenings, weekends, and holidays may be required; Participation in an on-call rotation.

Qualifications:

  • Ability and willingness to work in a diverse and challenging environment.

  • Flexible and passionate about providing a broad spectrum of services to vulnerable populations.
  • Strong written and verbal skills.
  • Bachelor’s degree in psychology, sociology, human services, or other related field; BSW/MSW preferred.
  • 2+ years experience with individuals experiencing homeless, mentally ill or substance abuse preferred (work or volunteer).
  • Proficient in Microsoft Word, Excel, and other Office Software
  • Experience with Homeless Management Information System (HMIS), a plus
  • Valid driver’s license and ability to operate a motor vehicle.

Program Responsibilities:

  • Provide comprehensive, participant centered case management to participants receiving supportive services through both HUD Permanent Supportive Housing and Shelter Plus Care contracts
  • Complete monthly home inspections to assigned participants residing in scattered site permanent housing placements
  • Develop, implement, and review individualized case plans with each participant to accomplish goals such as physical health, money management, development of social/community supports, daily living activities, etc.,  on a monthly basis
  • Provide basic need items and supportive services to maximize independence and wellbeing of each participant
  • Provide budgeting, financial literacy, and Representative Payee services to participants to support money management for coverage of housing and other daily needs
  • Actively engage with participants by establishing trusting relationships, and assess for safety/well-being, and activate emergency services as needed.
  • Oversee medication management for assigned participants, as needed
  • Complete all HMIS documentation, data collection, and accurate case notes for all services rendered
  • Participate in Coordinated Entry Assessment through the completion of required VI-SPDAT, SPDAT, and TAY-SPDAT assessments.
  • Provide person centered supportive services through the Welcome Center Drop-In services, as needed.

For Friendship Place:

  • Participate in the planning and development of new projects as they relate to participant services
  • Assess safety/well-being of participants and activate emergency services and other necessary supports as needed.
  • Connect, collaborate, build, and maintain positive relationships with external agencies that provide services to Friendship Place consumers.
  • Actively participate in case meetings, staff meetings, and supervisions.
  • Actively participate in trainings and other events offered by Friendship Place and other agencies.
  • Assists with volunteers as necessary.
  • Perform other duties as assigned.

Specific Requirements:

Employee must have his/her own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured, registered, etc.) and the employee must have a valid driver’s license.

Background Checks: The selected candidate will be required to pass a criminal history background check and/or fingerprinting including FBI, MPD, CFSA, and drug-testing.

 Other Details:

• Compensation-The hiring pay range is between $46,228 – $49,044 annually. Depending on the position, the starting hourly pay rate or annual salary is based on a step system. A candidate’s starting pay determination is made based on meeting the basic position requirements, and current completed education, credentials and/or work experience. Please Note: The step system is set and not open to pay negotiation. Before moving forward in the selection process, interested applicants are encouraged to discuss starting pay eligibility with the Human Resources (Talent) Department.
