Full-Time Practice Transformation Specialist

What You Will Be Owning:
As a Practice Transformation Specialist you will be responsible for project management within our Accountable Care Organization (ACO) within Colorado for a given group of providers whom the PTS provides on-site, as well as distance support, as providers work to complete Aledade ACO projects. This work requires frequent visits to your group of practices in order to personally assist the physicians and staff with practice transformation plans.
Professionals who have experience in the following roles (or similar) are very well suited for this position: Case Managers from hospital or physician practice market, office managers or staff nurses from the physician practice market, ambulatory care clinic manager, home care nurses, social workers with experience in long term care or institutional facilities.
We are looking for someone who can:
Identify and develop suggestions for provider participant’s improvement plans and assist providers in developing, implementing, monitoring, and tracking of improvement activities.
Utilize professional clinical and quality improvement experience and expertise to guide the project team.
Work directly with the ACO Coordinator to refine and maintain project-specific timelines and communicate the projected timeline to all other necessary Aledade staff that will be impacted by project activities.
Clearly articulate and communicate to the Aledade leadership and other project team members’ areas of concern, needed tools, or barriers to achieving goals of the ACO.
Partake in proactive communication and problem solving with the practice for efficient and effective progression of the ACO initiatives.
Partner with the Executive Director and ACO Coordinator in identifying and supporting ACO related activities with the practices served
Access and incorpore available resources, tools, and/or materials whenever possible to support the Aledade strategy.
Facilitate development of a goal-oriented plan for interim monitoring of process / lead and outcome / lag measures.
Assist in the preparation of proposals, reports, updates, and summaries for all assigned projects within designated time frames.
Participate in sharing and learning related to Aledade ACO initiatives and other similar activities.
Identify opportunities for improvement in the operations of the HIT Team and other processes in which team members are involved.
Understand Resource Availability: tech, analytic, initiative and field assets for how specific savings drivers fit into big picture goals of the ACO.
Maintain a working knowledge of other healthcare related initiatives, cooperate and encourage collaboration to share resources, tools and materials whenever possible.
You have a degree in nursing or social work with experience in a clinical setting is preferred. Graduate of an accredited college or university in health technology or information technology-related program accepted.
You may have significant and relevant work experience in office practice management in lieu of educational requirements may be accepted, particularly with significant administrative experience in a clinic setting.
You are located in Colorado. Travel to Aledade offices will be required.
You are comfortable working in a fast-paced, dynamic environment while maintaining focus on key goals.
You have sound clinical knowledge and familiarity with the Colorado health care delivery system.
You have excellent organizational skills.
You have excellent oral and written communication skills (i.e., public presentation skills).
You have exceptional interpersonal communication and team-building skills.
You have the ability to manage multiple projects and activities on a routine basis, with minimal supervision.
You have demonstrated knowledge of continuous quality improvement techniques.
You are familiar with process mapping and work-flow analysis tools.
You have experience with clinical process or performance improvement and technical skills related to EHR implementation.
You have excellent computer skills and willingness to learn additional software applications.
You have the ability to travel up to 50-75% of the time within Colorado and an occasional overnight travel.
