Central City Concern’s (CCC) Blackburn Center is a new six-floor building in East Portland, Oregon (clinic and housing) that will give vulnerable people access to integrated housing, employment and health services, and provide support they need to stabilize and rebuild their lives.
CCC is hiring employees to help lift this project to national prominence. Known for innovative programming already, CCC now moves toward a first-of-its-kind operating model (including key changes such as centralized intake and integrated care teams) in a more diverse community and a landscape of new potential community partners.

TITLE: On-Call Health Assistant
DEPARTMENT: Blackburn Center
REPORTS TO: Care Team Manager
SALARY: $17.00-$18.70 hourly
SCHEDULE: On-Call. No set schedule. On Call employees must work at least once in every 6-month period to remain active on payroll.
SUMMARY: The Blackburn Center is an innovative program that will offer integrated housing, health care, and employment services with the long term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations.
The Health Assistant (HA) works as a member of a multi-disciplinary primary care team providing integrated primary care services to homeless and low-income patients seen at the Blackburn Center. The Care Team is responsible for proactively managing the healthcare needs of the patients on their assigned panel, including behavioral and medical health needs. The HA is responsible for providing support to the Care Team Managers (CTM) and providers on each primary care team.
1. High school diploma or GED required. Prefer Bachelor’s Degree or higher in health or social service related field of study.
2. At least one year experience working with homeless, low-income, or other vulnerable populations in a supportive role preferred.
3. At least one year experience working in a clinic setting preferred.
4. Bilingual preferred.
5. Has the competencies necessary to:
a. Communicate effectively orally and in writing;
b. Understand healthcare related terminology;
c. Provide outreach, engagement and supportive services
6. If a recovering chemically dependent person, must have 2 years of continuous sobriety from date of employment.
7. Must have current CPR certification or must be able to obtain in one month.
8. Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
9. Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
10. Must adhere to agency’s non-discrimination policies.
11. Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles and sexual orientations, and treat individuals with respect and dignity.
