Full-Time Food and Transportation Programs Coordinator

Job Description

Under the direction of the Director of Social Services, the Food and Transportation Programs Coordinator coordinates, monitors and evaluates the delivery of food and transportation services to the clients and patients of Desert AIDS Project (DAP), in a manner consistent with the policies and procedures of DAP and related program protocols.

FLSA Status: Hourly, Non-exempt

 

Reports to:  Director of Social Services

 

Essential Functions

  • Ensure that Food and Transportation program’s goals and objectives are met as per the scopes of work, grant requirements, and DAP protocols;
  • Ensure that food and transportation services staff has the skill, knowledge, resources and tools to adequately assist clients in receiving timely and coordinated access to these services;
  • Facilitate a collaborative, coordinated interface with Medical Case Managers to ensure the appropriate delivery of food and transportation services to clients/patients in a timely manner;
  • Maintain database food and transportation programs in the community appropriate for our clients/patients;
  • Process billing requests for food and transportation vendors;
  • Serve as a DAP Liaison for food, transportation agencies in the community;
  • Ensure that Food Depot pantry complies with sanitizing, pest control and safety requirements from the Department of Environmental Health; to have on file a current Food Handlers Certificate and attend Food Safety Classes;
  • Work with food and transportation providers (governmental and non-governmental agencies, individuals and corporations) to ensure a supply of food and transportation services to meet the needs of the clients;
  • Ensure accurate and timely reporting of food and transportation services data for grant reports, contracts and any other service reports;
  • Attend mandatory meetings at F.I.N.D., HIV Planning Council Committee meetings (ad-hoc sessions) among others as needed;
  • Maintain accurate records of all client interactions in client services database in a timely manner;
  • Implementing new or revised guidelines in an n effort to eliminate duplication of services on the part of various providers throughout the Coachella Valley;
  • Participate and coordinate Food and Transportation response to grants and contracts audits throughout the year;
  • Continuous monitoring of budget expenditures to keep in line with budget parameter dictated by grants and contracts;
  • Coordinate the ordering, delivering, and stocking of food items at the Food Depot;
  • Monitor all food purchase and food donations, to verify that we are in compliance with the Department of Environmental Health, for annual in site inspections;
  • Coordinate the pick-up and delivery at F.I.N.D. for distribution day;
  • Coordinate volunteers assigned to Food and Transportation Programs;
  • Perform other job-related duties as assigned.

Required Education and Experience

  • Graduate from a high school, or possession of a certificate of proficiency issued by the California State Board of Education, or possession of a G.E.D. certificate;
  • Minimum one year work experience in social work, medical or social services, or related fields where job duties included access to confidential client records and customer service;
  • Strong computer skills including proficiency with in working with Microsoft Outlook Suite.

Preferred Education and Experience

  • AA college degree;
  • Experience in program service management;
  • Experience in the delivery of Ryan White or grant funded programs within a social services setting.

 

License(s) and Certification(s)

  • Current American Heart Association or American Red Cross accredited BLS certification;
  • Current Food Handlers Certificate issued by the Riverside County Department of Environmental Health.

Working Environment and Physical Demands

This job operates in an office setting and requires frequent times of sitting, standing, repetitive motion and talking. Ability to lift up to 25 pounds. No direct patient contact under normal circumstances is required. No exposure involvement to blood, body fluids, and non-intact skin or tissue specimens required. Access to patient protected health information may be required.
