Job Description

Implement High Fidelity Wraparound model to coordinate care and improve the overall health and well-being of multi-system involved youth and their families

·    Assesses, plans and implements social work strategies to improve the health and well-being of individuals and families.

·    Participates in interdisciplinary team meetings.

·    Maintains cooperative working relationships among public and agency participants.

·    Provides consultation to and works with community groups.

·    Facilitates collaboration among varied community partners.

·    Plans or participates in development of program proposals for funding.

·    Performs job duties under the regulations, policies and procedures set forth by Summit County Public Health and outside agencies such as the County Fiscal Officer, Ohio Department of Health, and state and federal government.

·    Performs any other duties as required for the efficient operation of Summit County Public Health.

·    Obliges, as set forth by the Summit County Public Health Emergency Operations Plan and within the scope of their employment, to be available for emergency service during routine and non-routine hours of operation including 24/7/365 availability.

Required Qualifications

  • Baccalaureate Degree in Social Work or related field.
  • Knowledge of community resources designed to support youth and families.
  • Understanding of and experience with different child-serving systems, including education, behavioral health, child welfare, juvenile justice, developmental disabilities, health and others.
  • Experience in building and maintaining collaborative, professional working relationships with community partners for the expressed purpose of meeting the needs of youth, families and communities.
  • Experience with multisystem, multi-need individuals and families, treatment planning, coordination of services, linking services, and record keeping.
  • Current valid Ohio driver’s license, automobile availability and insurance coverage.
