Full-Time Community Case Manager – Brooklyn TASC

Job Description

EAC Network seeks a Community Case Manager for the Brooklyn Treatment Alternatives for Safer Communities (TASC) Program, located in Brooklyn, NY. The position is full time, Monday-Friday 9:00AM-5:00PM with excellent benefits medical/dental/401K and very generous Paid Time Off (PTO). 

Primary Purpose of Job:

The Community Case Manager monitors participants time in the community.

Principal Duties & Responsibilities:

  1. Conduct assessment interviews and arrange for clients to be placed into the appropriate treatment program. Investigate and arrange client service plans.
  2. Complete follow-up paperwork and maintain complete client records and have them available for inspection by Supervision at any time.
  3. Prepare and present written status reports to the criminal justice system in a timely manner regarding client’s progress.
  4. Escort clients to appointments and treatment facilities when indicated.
  5. Investigate and arrange supportive services for clients and conduct regular calls and random home, school, and work checks and contact treatment providers to follow up on services provided and verify participation in prosocial activities and tasks.
  6. Make monthly site visits to treatment agencies.
  7. Conduct drug testing on clients.
  8. Conduct cognitive behavioral interventions.

Knowledge, Skills, & Abilities Required:

  1. Associates degree in a helping profession (educational equivalent will be considered) and at least 2 years of prior experience working in intervention programs with criminal justice involved participants. Bachelors degree preferred.
  2. Knowledge of treatment programs and local criminal justice and health systems.
  3. Good communication skills, both oral and written; Ability to work cooperatively with treatment and criminal justice systems.
  4. Must be willing to travel throughout NYC.
  5. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
