Full-Time Case Manager Aide / Peer Mentor – Staten Island TASC

Job Description

EAC Network seeks a Case Manager Aide / Peer Mentor for the Staten Island Treatment Alternatives for Safer Communities (TASC), located in Staten Island, NY. The position is full time, Monday-Friday 9:00AM-5:00PM with excellent benefits medical/dental/401K and very generous Paid Time Off (PTO).

Primary Purpose of Job:

The Case Manager Aide / Peer Mentor will assist the Program Director in a wide range of administrative and support related tasks to ensure program priorities are maintained.

Principal Duties & Responsibilities:

  1. Coordinate and lead peer support groups and provide on-going support for recovering clients settling into community life by active collaboration with case managers. Interface with health system on behalf of clients.
  2. Participate in planning for services for clients, through virtual and face-to-face meetings and through participation in team discussions where client status is discussed.
  3. Advocate for and actively support efforts to assist clients in areas that may not be adequately addressed by the community provider responsible for treatment.
  4. Monitor client satisfaction with services and apprise case management staff.
  5. Maintain client charts and notes, update after each interaction with clients, treatment providers, family members, etc.
  6. Escort clients to treatment facilities, hospital, crisis centers, and other services when indicated and assist in connecting clients to community-based peer support resources.
  7. Conduct supervised toxicology testing as needed.
  8. Interact with SCAM program participants in a professional manner and educate them on the program and requirements.
  9. Install SCRAM monitoring device on program participants and download SCRAM data for reporting.
  10. Collect and monitor payments under the supervision of the Program Director.
  11. Assist with reconciliation of program funds for deposit under the supervision of the Program Director.
  12. Assist in managing SCRAM bracelet and accessory inventory to insure they are available for participants and in proper working condition.

Knowledge, Skills, & Abilities Required:

  1. High School diploma or educational equivalent required. Completion of a Peer Training Program preferred.
  2. Ability to work cooperatively as part of a team.
  3. Excellent communication, organizational and interpersonal skills.
  4. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments
