Full-Time Case Manager, Adults – SOJOURN

Job Description

Schedule: Monday – Friday (9:00am – 5:30pm); occasional evenings and weekends as needed

SUMMARY

One of the oldest domestic violence shelters in the state, Sojourn provides shelter and comprehensive services to victims of domestic violence and their families. Sojourn staff implement trauma-informed, empowerment and housing first philosophies to ensure the health and well-being of survivors and their children, and work with families to find safe housing alternatives. 

ESSENTIAL VDUTIES & RESPONSIBILITIES

Under the supervision of the Shelter Manager, the Adult Case Manager will:

Case management:

  • Provide case management (housing, medical & social service advocacy, job development, transitional housing, etc.)
  • Provide on-going peer counseling to all adult clients living in the shelter
  • Interface with other organizations to gain resources that can benefit adult clients
  • Provide possible co-case management with other The People Concern projects/ outside agencies
  • Conduct intake/assessments/health screening of adult residents within the first week of their arrival
  • Create safety plans, housing plans, and emergency plans for every adult client in shelter
  • Provide hotline coverage and Conduct second assessments as necessary
  • Facilitate any additional programming component(s) or multidisciplinary teamwork proposed for the program, as directed
  • Grocery shopping for house and special events
  • Reliably provide scheduled coverage;
  • Cover for shelter team members out sick or on vacation or in an emergency
  • Train volunteers on the hotline/ house coverage

Healing Groups, Trainings and Meetings

  • Attend trainings as directed or necessary to perform Case Manager duties well
  • Design and Facilitate wellness & healing groups to aid in the trauma healing process
  • Develop and teach Nutritional/Healthy cooking/eating to promote growth and physical health
  • Facilitate weekly Caseload meetings
  • Provide parenting skills training to shelter residents in conjunction with Children’s Case Manager as relevant
  • Assume a Leadership role in Project GROW, and assist in maintaining gardens, keeping records, reporting statistics/narrative
  • Train new staff on hotlines/house coverage
  • Attend and actively participate in weekly shelter program meetings, weekly caseload meetings, full-staff meetings and Sojourn/The People Concern retreats

Data & Documentation

  • Scrupulously maintain case notes, shift change logs and other shelter records
  • Write and report narratives, provide data and success stories on monthly, quarterly or annual basis, or as directed
  • Advise hotline staff regarding possible clients coming into shelter
  • Ensuring all grant requirements for Adults are met
  •  Maintain extensive file of printed program and educational materials with responsibility to expedite translation of materials into Spanish
  • Create and maintain physical and electronic files for all adult clients

Other Duties

  • Responsible for safe storage and responsible and ethical use of all donated or purchased materials
  • Required to lift and carry heavy bundles and items, donations and residents’ belongings
  • Required to transport clients and clients’ belongings
  • Occasional court accompaniment / translation services
  • Plan special events for adult clients birthdays &  goodbye ceremonies
  • Responsible to assist other staff in maintaining a culturally sensitive shelter environment
  • Use of own vehicle; responsible to carry all auto insurance legally required in State of California
  • Required to be fingerprinted and screened for tuberculosis
  • Other duties as assigned
QUALIFICATIONS
  • BA or other college degree required;
  • Minimum of two years’ experience in program or case management required; 24-hour shelter experience preferred
  • In-depth knowledge of domestic violence issues and politics preferred;
  • Culturally competent;
  • Developed skills in crisis intervention, peer counseling, trauma-informed care, and service delivery;
  • Familiarity with statistical reporting, data collection, and grant and contract management;
  • Proficiency in MS Office (Excel, Word, PowerPoint); (testing provided)
  • Ability to lift, carry, and transport heavy bundles and items, donations and residents’ belongings;
  • Pass Live Scan background check and tuberculosis screening;
  • Driving is an essential function of the position:  must have a currently registered and insured vehicle in good working condition and a valid CA driver’s license.

Work Environment

  • Combination of either field and/or office environment depending on position
  • On occasion walk or drive to different agency sites
  • May/Will necessitate working in busy and loud environments depending on position
  • May/Will be exposed to elements like cold, heat, dust, noise and odor depending on position
  • May need to bend, stoop, twist and sit throughout the day
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
