Job Description

We are currently seeking a Care Management Team Leader-Health Home to work at our 400 Broadway location.

 

Position Summary:  The Care Management Team Leader provides supervision to Care Managers staff by chart review, face to face case discussion and performance reviews; Meets performance goals set by Apicha Support Service Department; Coordinates and oversees the services among care managers as well as the implementation of the care plan and plan of care.  The Care Management Team Leader maintains client-related records and other requirement documentation as well as conduct quality assurance activities on a regular basis.

 

 

Qualifications:

Qualified applicants will have a BA/BS in nursing, health or human services field with five (5) years of experience in related field required. Advanced degree in related field with 3 years of experience preferred; Experience supervising clinicians and/or case workers who are providing direct services to individuals with severe mental illness/serious substance abuse preferred; Experience and/or knowledge of NYS Health Home preferred; Experience in working with one or more of the following communities is required:  persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse, chronic disease including HIV/AIDS; Ability to communicate in a warm, caring manner and to motivate others with professional demeanor; Fluency in Spanish or one of Asian languages preferred; Excellent written and verbal communication skills in English required.
