Full-Time Care Coordinator (HARP) – Suffolk Health Home

Job Description

EAC Network seeks a Health and Recovery Plans (HARPCare Coordinator for the Suffolk Health Home Program, located in Hauppauge, NY. The position is full time, Monday-Friday 9:00AM-5:00PM with excellent benefits medical/dental/401K and very generous Paid Time Off (PTO).

Primary Purpose of Job:

The Health and Recovery Plans (HARP) Care Coordinator conducts outreach, assessment, and service planning to coordinate care for participants with severe and persistent mental illness who have been identified as HARP-eligible. Conducts assessments and prepares a plan of care by working closely with clients, managed care and all medical and Home and Community-Based Services (HCBS) providers. Maintains contact with the client’s healthcare team, family and support networks for coordination of care; Monitors client’s progress in utilizing services to achieve better health care outcomes while containing costs.

Principal Duties & Responsibilities:

  1. Gather information for and or conduct outreach, intake, assessment, and reassessments.
  2. Provide care management to a caseload through the coordination of medical, mental health, HCBS, and substance abuse services independently with minimal supervision.
  3. Conduct assessments and prepare a plan of care as directed by NY State and Managed Care Organizations.
  4. Collaborate with the individual’s HARP team including, MCOs, HCBS, medical and treatment providers.
  5. Ensure entitlements, insurance, and benefits are in place.
  6. Develop service plans and resolve barriers to effective service utilization.
  7. Monitor client’s progress in utilizing services (appointments, treatment, medication, etc.) through telephonic and direct contact.
  8. Check that clients receive test results and track that clients follow up with medical directions.
  9. Escort clients to/from medical appointments when needed.
  10. Collaborate on care issues with internal clinicians and the participant’s assigned Health Home medical provider.
  11. Document service information in databases and use electronic systems to track participant activities.
  12. Assist in crisis intervention and provide or refer to crisis services.
  13. Conduct home visits and community work in the field including visiting hospitals and emergency rooms when determined necessary.
  14. Ensure that clients follow-up with aftercare discharge (i.e. fill prescriptions, make appointments).

Knowledge, Skills & Abilities Required:

  1. Bachelors Degree in social work, psychology, sociology, community health or a related field required.
  2. Minimum of two years experiences providing mental health services, social services or substance abuse-focused services to individuals with complex behavioral health conditions required.
  3. Knowledge of health care environments, clinical terminology and health information systems strongly preferred.
  4. Must have valid NYS Driver’s license and be willing to transport clients to appointments using personal vehicle if needed.
  5. Excellent organizational and time management skills, interpersonal skills, verbal and written communication skills required.
  6. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments
