Full-Time PrEP Navigator

Job Description

1.Identify and plan effective intervention strategies to increase the number of individuals at high risk of HIV infection who are engaged in prevention services.

2.Work closely with colleagues from ACG and other community-based organizations to design and schedule community outreach initiatives and screening events in bars or non-traditional outreach settings, including social media, to pro-actively find and assist LGBTQ/MSM persons at risk for HIV/AIDS.

3.Work with ACG’s STI testing and treatment teams to strengthen the coordination and referral system for at-risk participants in need of clinical services in order to increase referrals and facilitate care for participants.

4.Responsible for day-to-day coordination of patient activities including coordination of referrals for rapid testing and treatment, identification of eligible patients, and timely enrollment of patients in the treatment support and prevention programs.

5.Tracks inter and intra-departmental enrollments, patient progress, and dis-enrollment, reporting data to funders and quality committees, as requested.

6.Gathers and submits information needed to obtain Prior Authorization for PEP/PrEP, and other medications.

7.Completes applications and collects and submits documentations as needed for medication Patient Assistance Programs.

8.Assists in referral to support services as ordered by the medical provider.

9.Performs outreach in the form of appointment reminders, no show follow-up calls, prescription updates, community health events, educational and testing events at partnering facilities and field recruitment.

10.Attend continuing education seminars, staff in-services and stay abreast of relevant literature to maintain familiarity with trends in prevention services

11.Participate in program planning through interdisciplinary team meetings.

12.Establish, build and maintain partnerships in the community with collaborating agencies or other venues with a focus on those that serve hard-to-reach or disproportionately impacted persons at risk for HIV/AIDS (such as bars, nightclubs, public sex environments, shelters, Pride events, etc.) and through social media to promote risk-reduction activities and to connect participants to care.

13.Other duties as assigned by program supervisor.

Qualifications:

At least three years’ experience working with high-risk populations including MSM and LGBTQ individuals in HIV-related programming.
Knowledge of priority populations at risk for HIV and STI transmission through high-risk sexual behaviors. Knowledge of HIV/AIDS and Harm Reduction principles and practices.
Experience planning and utilizing diverse outreach methods, including online outreach, to effectively reach high-risk populations including IV drug users, MSM and LGBTQ and populations at risk.
Experience or knowledge in health insurance applications.
Excellent verbal and written communication and interpersonal skills
Strong computer and analytical skills; substantial experience in Windows computer applications
Ability to be creative and have a flexible schedule to adapt to the needs of the community, program, and participants
Valid Driver’s License
